This section is designed to help you better understand your financial responsibility for the services provided by Mayfield physicians and nurse practitioners.

Depending on whether you have insurance, and the extent of your coverage, you may be asked to make a payment at the time of your visit. We suggest that you contact your health insurance provider before your appointment to verify benefits and coverage and to confirm that your Mayfield physician is a participating provider.

Payments
Co-payments and payments for services not covered by an insurance company are due at the time of service. Please remember to bring your insurance card, Medicare or Medicaid card with you to your office visit. If you do not bring your insurance card, we will ask that you pay in full at the time of your visit. For your convenience, Mayfield accepts cash, personal checks, Visa, MasterCard and Discover.

Health Plan or Managed Care Organization
If you are enrolled in a health plan or managed care organization (HMO or PPO) that includes your Mayfield physician as a participating provider, your co-payment will be collected before you see the doctor.

Medicaid
If you are enrolled in Medicaid you are not required to make any payment at the time of the office visit. However, if you do not have your Medicaid card with you at the time of service, we will ask that you pay in full at the time of your visit.

Medicare
If you are enrolled in Medicare and do not have secondary insurance coverage, your 20% co-insurance will be collected at the time of your visit. Conversely, if you have secondary insurance coverage to Medicare, you are not required to make any payment at the time of the office visit.

Once these charges have been billed and benefits determined by the insurance plan, you will receive a statement from Mayfield Clinic detailing your account activity and financial responsibility, if any. If you do not have your Medicare card with you at the time of service, we will ask that you pay in full at the time of your visit.

If you are enrolled in a Managed Medicare Plan, your co-payment will be collected before you see the doctor.

Worker’s Compensation and Self-Insured Employer
If you are being seen for a work-related injury, your claim must be approved by the Bureau of Worker’s Compensation or self-insured employer. If approved, you will need to provide the following information: date of injury, claim number, the managed care organization that administers your benefits and billing address.

If your claim is pending or not approved, Mayfield will submit a claim to your health insurance company. If you do not have health insurance or you do not provide your Worker’s Compensation number, we ask that you pay in full at the time of your visit.

Out-of-Network Insurance
If the Mayfield Clinic is not included on your insurance plan, we ask that you pay in full at the time of your visit.

No Insurance
If you do not have health insurance, we ask that you pay in full at the time of your visit. Please contact Mayfield’s financial counselors at 513-569-5376 or 513-569-5270 if you have questions.

Refunds
If you have any questions concerning a refund on your account, please contact Mayfield's Patient Accounting Department at 513-569-5300 or 800-325-7787.
