Request an MRI scan

Referring physicians and their staff members can request to schedule an MRI at the Mayfield Imaging Center by completing an online appointment referral form.

MRI Referral Process

1. Complete and submit Referral Form
   
   
2. Complete the insurance precertification form (.pdf) and fax with copy of insurance card (front and back) to 859-341-1983
   
   
3. MIC will contact the patient and schedule the MRI
   
   
4. MIC will perform the insurance precertification
   
   
5. MIC will fax you an appointment confirmation
