neurotrauma

The Neurocritical Care program is based out of University Hospital under the direction of Lori Shutter, MD. The fellowship focuses attention on training physicians in the acute medical care of critically ill patients with an underlying neurological process. Specific diseases and conditions include but are not limited to: cerebrovascular disorders, neurotrauma, neuro-oncology, refractory seizures, neuromuscular diseases, infections, alterations in consciousness, and perioperative neurosurgical care. Clinical training is a minimum of 2 years with 12 months of direct management of ICU patients. In addition, there are rotations in other ICUs and anesthesiology. Although preference will be shown to applicants with training in neurology, neurosurgery, or emergency medicine, applicants trained in other specialties will be considered.

 

Clinical services are provided in the Neuroscience Intensive Care Unit (NSICU). The NSICU is a 20-bed state of the art unit with full cardiopulmonary monitoring capabilities, specialized neuro-monitoring options, transcranial dopplers, continuous EEG, and computerized radiology linkage.

Educational Goals

  • The Fellow will gain expertise in the clinical management of patients with disorders or injury to the brain and/or spinal cord including:

    1. Initial resuscitation and emergency treatment/diagnostic evaluation.
    2. Critical care management and treatment of neurological conditions.
    3. Transitioning patients to the neurological rehabilitation phase of their treatment.

  • The Fellow will gain knowledge and understanding of the collaborative interaction between neurosurgeons, neurologists, neuroradiologists, emergency medicine, and trauma surgeons.
  • The Fellow will gain a working knowledge of the physiology and pathophysiology of neurological conditions.
  • The Fellow will participate in available opportunities for clinical and/or laboratory research in ongoing and/or newly developed studies.

Duties and Responsibilities
The Neurocritical Care Fellow will participate as an active member of the NSICU team. He/she will:

  • Participate in the daily operation of the NSICU and reporting directly to faculty responsible for the service.
  • Participate in the assessment of patients including clinical examination and diagnostic procedures.
  • Participate in diagnostic procedures including radiologic and physiologic studies.
  • Participate in the interpretation of the above data leading to the decision making process, ultimately developing a treatment plan.
  • Participate as an active member of the NSICU team, enabling him/her to gain the essential procedural experience to provide critical care efficiently and effectively.
  • The Fellow will play an active role in ongoing clinical research protocols. In addition, the participant is expected to develop an independent clinical study that he/she will bring to fruition ultimately submitting a clinical protocol to the Division Director and/or submitting the results of an independent study to a peer review journal.

Evaluation Process
The Fellow is evaluated semiannually by the Director of the Neurocritical Care Fellowship program and members of the team participating in the care of patients on the NSICU.
